About this page. Clinogistic is a clinical operations platform used by licensed healthcare clinics to communicate with their patients. This page shows the exact consent language and opt-in flow a patient encounters when they register at a clinic that uses the Clinogistic platform. SMS consent is collected at patient registration, either via the digital intake form shown below or via a paper equivalent, with the same disclosures.

How patients opt in

When a patient registers at a Clinogistic-powered clinic, they complete an intake form. Part of that intake form asks for explicit SMS consent. Consent is voluntary and unchecked by default. A patient may decline SMS consent and still receive all clinical services. The exact form a patient sees is reproduced below.
